Hmong Health Collaborative Grant

Funding assists nine Hmong serving nonprofit organizations in the Central Valley from Fresno to Sacramento to collaborate and advocate for Hmong community residents.

 

 

HWISH Grant
Provide a diabetes awareness program, screening, and intervention to Hmong elders within the two Central Valley counties of Merced and Sacramento.   A target of 750 Hmong elders will be screened for diabetes and those whose blood sugars are out of the normal range will be provided with follow-up education and referral for primary care to ensure they receive necessary treatment.  Moreover, these identified elders will also receive ongoing case management throughout the period of the grant to track their access to and implementation of treatment and lifestyle changes which will impact the management of the disease and, ultimately, blood sugar control.

 

 

Going Places

Provide interpreting services for children 0 – 5 and pregnant women needing out of county specialty services in collaboration with Merced County Department of Public Health and VIA Adventures transportation company. If you are in need of transportation for your child age 0 to 5 or you are pregnant please contact Healthy House at (209) 724-0102.   We can refer you to Public Health and provide interpreting services if needed.



Interpreter Training

Provide annual convenings for interpreter trainers, continue state and national involvement in healthcare interpreting field, and establish a 288-hour interpreter certification program at Merced College. 



Language Bank Services

Provide interpreting service in Spanish, Hmong, Mien, Lao, Portuguese, Punjabi, Hindi, Chisene and Cantonese.



Language Proficiency and Interpreter Readiness Assessments

Provide language proficiency tests in Spanish, Hmong and English and assess interpreting skills.  



Translation/Transadaptation Services

Provide meaningful message conversion in Spanish, Hmong, Punjabi and Hindi.  



Training

Examples of trainings include:    Working Effectively with Interpreters, Cultural Humility Trainings, Unnatural Causes, and Leadership Development in Interethnic Relations. 



Partners in Healing

Funded through Mercy Medical Center Merced to provide quarterly facilitated tours for Hmong shamans to inpatient and outpatient services to improve understanding. 



Mental Health First Aid

12 hour training program about common mental health disorders and an action plan to help people get support and access help early.  We also offer similar trainings in Spanish and Hmong based on the Mental Health First Aid model.


MATCH (Multidisciplinary Approach to Cross-Cultural Health) Coalition- Unfunded, co-facilitate monthly meetings about different of health related problems of service.
